Who We Support
At Care Solutions Hub, we provide tailored support to:
- Individuals with a primary diagnosis of Learning Disabilities and/or Autism Spectrum Disorder.
- People aged 16 and above.
- Those residing in Surrey, London, Dartford, and Hampshire.
What Happens When You Make a Referral?
When you refer someone to Care Solutions Hub, we review the referral to determine if we have the capacity to offer support. If we are able to assist, the next step will be to schedule an initial assessment.
During this assessment, which typically lasts 1-2 hours, we will meet with you and anyone important to you to explore your needs and the type of support you require.
We’ll discuss topics such as:
- Your diagnosis and medication.
- The support you’re looking for.
This is also a chance for you to ask us any questions.
After the assessment, we will decide if we are able to support you and liaise with your social worker or case worker to move things forward.
